Transport for NSW (formerly RMS) Driver Vision Forms
Lifestyle Optical Optometrists are registered to complete and submit the Transport for NSW Driving Vision Reports online.
If you choose to renew your Driver License online, you will receive a letter from Transport for NSW to visit an Optometrist or Ophthalmologist to perform the eyesight component within six weeks of the date of the letter.
Once submitted you will receive a copy of the certification by email from us for your records only. You do not need to contact Transport for NSW after your eyesight test.
